Reference frameworks provide common reference to healthcare professionals to facilitate the provision of continuing, comprehensive and evidence-based care in the community. Through the reference frameworks, it is also intended to empower patients and their carers, to raise public awareness of the importance of proper prevention and management of chronic diseases, as well as conducting health promotion and disease prevention for different population groups.
