Transitional Support Service for Persons in Mental Recovery (CSDI Portal)

Transitional Support Service for Persons in Mental Recovery provides a stop-gap, timely and enhanced transitional support to service users to be discharged from hospital and waitlisted for halfway house service. Through medical-social collaboration, the clustered-based multi-disciplinary outreach support teams will help the service users to smoothly transition into halfway house service. Also, by strengthening support to their families/carers, service users' individual needs amid their recovery pathway will be met and they may reintegrate into the community.
